2010 QMA APPLICATIONS

The 2010 QMA applications are available.  The new applications have 2 pages, both of which need to be completed.  There is also an Adult Waiver and a Minor Waiver that needs to be signed by each handler (adult waiver) and each driver (minor waiver).  Please do not sign these applications unless you are in front of a club officer or you may use a Notary Public.  If you choose to sign before a club officer, please make sure they put the QMA embossed stamp on the application and waiver(s).

Here are a couple of reminders about QMA Memberships:

A.     Regular Memberships - are family memberships including husband, wife and all
        drivers under the age of 18 years old and a member of that immediate family
        who all live in the same household.

B.     Alternate Handlers - are not included in a family membership unless they live in
         the same house and are at 16 years of age.

Once you have completed the two-page application and all applicable waivers (unsigned, unless using a notary public), please submit the following to an officer:

1.        QMA Application (both pages)
2.        Adult and/or minor waiver(s) (both pages)
3.        Yearly Fee ($150-Regular and $100-Alternate Handlers) made payable to
           CCQMA
4.        Concession Fee of $80 (which can be earned back) made payable to CCQMA
5.        1x1 photo - For new adult members only
6.        Driver's birth certificate for all new drivers or alternate handler who
           resides in the same household as the handler

2009 CCQMA Banquet

Our banquet will take place on Saturday, December 5th at the VFW Hall in Taylorville (across from the track).  The club will provide a majority of the food but are asking club members whose last name begins with A to M to bring a side dish.  Members whose last name starts with N to Z please bring a dessert.  Also, the club will again have an auction.  Please begin to think about what item(s) you can bring.  This is a BIG money maker and goes towards next year's banquet.  Doors open at 4 p.m. and dinner is served at 5 p.m.  Cost is $5 for children 12 and under and $10 for individuals who are 13 and up.
